What is Penile Curvature?

Penile curvature refers to an abnormal level of curvature of the penile shape. This curvature is usually due to the abnormal development of the connective tissues surrounding the penile corpuscles or the disease in this tissue (Peyronie's).

Although the curvature is often seen upwards, it can be in different directions depending on the development of abnormal tissues. A normal penis should stand upright at a 90 degree angle during erection. Any angle disorder other than the condition in question is called penile curvature.

While penile curvature creates an aesthetically bad appearance, it can also cause problems such as difficulty in sexual intercourse or inability to maintain penile rigidity.

Apart from the sexual problems in question, it can cause the person to feel unhappy, deteriorate relations with the partner and cause depression. In Peyronie's disease, on the other hand, pain may be felt in the first stages, called the acute phase.

What Causes Penis Curvature?

Penile curvature is predominantly a congenital condition. However, it can sometimes develop due to various ailments, unsuccessful surgical applications or traumatic situations. If the condition is congenital, patients usually do not feel complaints such as pain. In the medical literature, penile curvatures are considered in two different categories: congenital and developing:

  • Congenital Curvature of the Penis: In the penis; There is 1 spongy structure in which the urinary tract is located and 2 spongy structures rich in blood vessels that provide erection. During the development of a man in the womb, a developmental disorder may occur in one of these two structures. In such a situation, the penis may curve towards the side where the development is less during erection.
  • Another cause of congenital penile curvature is hypospadias. Hypospadias is popularly known as the sunnah of the prophet. In this disease, the urinary hole is located below the point where it should be. With this disease, it is possible for the penis to bend in that direction because of less spongy tissue development or because of adhesions in the connective tissues of the penis.

  • Developing Penile Curvature: Later developing penile curvature is usually related to Peyronie's disease and is a calcification problem. Calcification occurs between the spongy structure in the blood vessel that provides the hardening of the penis and the outer sheath that provides the erection. This disorder, which usually occurs in advanced ages, manifests itself with pain, and deformity occurs later. In some patients, serious deformities, shrinkage of the penis, deformities and erectile dysfunction can be seen.

Penile Curvature Symptoms

While the symptoms that can be encountered in the case of penile curvature can sometimes be felt and seen, sometimes they can occur as a result of a physical examination by the physician. We can list these symptoms as follows:

  • Pain during erection,
  • Small and hard tissues on the back or body of the penis,
  • Erectile dysfunction or difficulty maintaining an erection
  • Visible curvature in the form of the penis,
  • Reduction in penis size,
  • Difficulty having sexual intercourse.

If the curvature of the penis develops due to the Sunnah of the Prophet, symptoms such as inability to urinate forward, scattered urination, frequent urination, difficulty urinating or feeling burning while urinating may occur.

Penile Curvature Treatment

Treatment of penile curvature is also done according to the cause of the curvature and the severity of the curvature. If the curvature of the penis is caused by the sunnah of the prophet, the only way to solve the problem is surgery. During this surgical procedure, the urinary hole is moved to where it should be, and if the curvature has developed, this situation is corrected.

In the acute phase of penile curvature (Peyronie's) disease that develops later, drug treatments and NSAIDs (anti-inflammatory) drugs or PDE-5 inhibitors can be used for the complaints of the disease. Shock wave therapy (SWT) can be applied in this phase.

In patients who have passed the active phase, the disease has not progressed for at least 3 months, and the penile deformity has not progressed, different treatments can be applied according to penile curvature-deformity and accompanying erection problem.

The curvature is corrected by surgery in patients with severe deformity and no loss of erection. Penile prosthesis is placed in the same session for those with concomitant erectile dysfunction. In patients who are not suitable for surgery or do not want surgery, drug injections (Clostridium histolyticum collagenase, interferon) can be applied to the plaque.
